HOW DO I DEFINE THE PERFECT LOOKING NOSE?
Clearly we all define beauty in our own way and it is part of my job to ensure I understand each patient’s desires for their appearance prior to surgery. But for me personally, I define a perfect looking nose in a female as one that is natural, that softens the features and simply brings out the other features of the face including the eyes and smile.
I am not sure I could have hoped for a better result than has been achieved with my patient, who has kindly allowed me to share her 3 months photos. She is ecstatic with how things have healed (as am I) and has been diligently following all my post op instructions about massaging and taping. I think this has all led to the near perfect result, at least in my eyes!
I will share her one week results in a video separately where she describes my post op routine.
REVISION RHINOPLASTY – ONE WEEK POST OP
This patient who had primary surgery elsewhere desired a more feminine profile and narrower appearance to the nose. The original surgery had not really gone to plan from her point of view.
She is only approximately 10 days post surgery and has had her cast removed. She had a beaming smile when I showed her to the mirror. I believe we have achieved everything she has wanted and whilst it is early, the indications are that she will get a beautiful result into the longer term.
We managed to take cartilage needed for the revision surgery from her nasal septum and hence there was no need for any rib to be used.
I am often asked whether revision surgery patients heal in the same way as primary cases. The answer is mostly yes albeit there are some more long term caveats. The immediate healing is similar but revision cases are more prone to scar tissue formation which can potentially cause more longer term consequences. I recommend various post operative massaging and taping techniques to reduce this risk.
NATURAL ENHANCEMENT
Everyone has different wishes about the shape of the nose but the majority of my patients seek a natural, unenhanced look that still keeps their identity.
This lady is now about 3 months post op and has achieved just that. She has a normal looking nose that just looks more refined and aesthetic without a dramatic change.
There is still some puffiness around the tip which will improve with time and injection of a little steroid into the area can help with this, particularly in patients with a thicker skin type.
I also encourage massage and taping in the post op period which further helps settle the swelling.
LOVELY CHANGES – 2 MONTHS ON
I was delighted to see my patient yesterday. She is still only 2 months post op following an open rhinoplasty to reduce the hump, narrow the tip and make the nose a little smaller overall, whilst maintaining a natural look.
She has healed remarkably well and already shows significant improvement. Further definition to the tip will occur with the passage of time as the skin swelling settles further.
She was absolutely delighted with her result. It appears to have increased her confidence immensely despite others not necessarily noticing that she has had surgery. This is an ideal scenario but she has still been kind enough to allow me to share her photos.
Things will get better and better and I look forward to watching the improvement in the months ahead. Seeing her so happy made my day yesterday and this is what keeps up my daily energy and enthusiasm!
ETHNIC NOSES
An early result ( approx 3 months) following a reduction rhinoplasty is an Asian patient. Ethnicity in patients does not necessarily change our patients’ wishes for the operation but the the skin thickness in particular can mean the goals for surgery need to be discussed to ensure management of expectations.
This patient kindly allowed me to share her photos at this stage. There js some swelling still around the tip region which will shrink further with time and improve the definition but she is delighted already with the natural appearance.
Thicker skin types will continue to shrink over a long period usually way beyond the one year we advise most of our thinner skin patients.
BEAUTIFUL PROFILE 5 MONTHS POST OP
I have only posted limited views of this patient at her request but she has a strikingly improved profile that very much is in balance with the rest of her face. It is a strong, natural appearance that will stand the test of time.
She is delighted (as am I) with the results and I wish we could have shown all of her near perfect post op results!

SHORTENING THE NOSE
The medical term for a nasal tip that sticks out a little too much is “overprojection”. This patient had an droopy and overprojected tip that needed attention. This was done by both rotating the tip up as well as bringing the tip backwards towards the face.
This is achieved by essentially shortening the cartilages that form the tip and is done in a strategic way to achieve both goals.
Clearly the hump on the nose similarly needed to be reduced to keep the whole nasal in balance. Overall a lovely, natural change which will transform this young lady!

PERFECTING A PROFILE
A female nasal profile ideally needs a little curvature without too much of a ski slope appearance. The tip needs to be slightly higher than the bridge, giving a supratip break which enhances the definition of the tip.
The aesthetics of this depends on the anatomy of each patient, their skin type and, of course, their own personal desires.
Our state of the art 3D digital imaging program helps patient understand what can be achieved. It provides a basis for which a decision can be made about the ideal individual profile based on all the above factors.

I was delighted to see this lovely patient of mine, now at 3 months post op. I have only shown the nose to protect her privacy at her request but the change is clearly transformative for her.
As always, things will improve further with the skin shrinking back with time but she is already delighted and it has given her a newfound confidence in her appearance, which was very apparent when we met last week.
3 MONTHS INTO A LIFE CHANGING TRANSFORMATION
Is anyone as enthused as I am about this gentleman’s result?
I featured some early on the table photos of my patient previously. He is now at over 3 months post op and things are beginning to look somewhere near where I will hope his eventual post op result to be. There remains a little swelling over the bridge of the nose that I could feel but this will shrink further with time, albeit relatively slowly, due to his thicker skin type.
Nonetheless, he really has had life changing surgery with a masculine, natural looking profile and a radical straightening of his tip. His breathing is massively improved as the septum was so deviated before too.
He has a new found confidence in himself and was kind enough to let me share his most recent pictures.
I would fully expect further improvements with time as the skin settles but he is delighted already (as am I) with his result.
MAKING A LARGE NOSE SMALLER
There are various considerations when trying to improve a larger looking nose. We need to consider how this might impact on the overall appearance relative to the rest of the face, any impact it might have on the function of the nose and also how the skin will shrink wrap onto the smaller framework. This is particular so in the thicker skin nose. Often patients want a significantly smaller looking appearance but I have to explain the limitations of how the thick skin will contract. Post op steroid injections, massage and taping also have a role to play. When planned and performed well, it can be a huge life changing experience

Types of Rhinoplasty We Offer

Reduction
The majority of our patients seek reduction in the size of the nose. This might involve taking down the hump of the nose as well as reducing the volume of the nasal tip to keep the nose in balance. Deviated Noses

Revision Rhinoplasty

As a result, the need for revision surgery is not uncommon. Our own revision rates are relatively low (about 5%) compared to published figures of other surgeons.
Having cosmetic surgery of any kind can be extremely daunting and to undergo corrective surgery to put right a previous rhinoplasty operation might be even more so. Revision rhinoplasty is most often performed to correct deformities caused as a result of prior surgery but may be required in patients unfortunate enough to have suffered trauma following prior rhinoplasty surgery.
Patients may unhappy with the look of the nose following their initial procedure and it can also impact upon the breathing. It is, however, critically important that the surgeon feels that any further surgery would improve the current situation and gaining trust is critical.

Due to the international reputation of our Medical Director, Mr Santdeep Paun, we have a lot of patients from abroad seeking our care.
We are able to provide highly individualised care packages for our overseas patients. Preliminary consultations can be had using Zoom or Teams or other such video messaging services although it is always difficult to assess patients properly in this way and we would insist on an eventual face to face meeting.
We can schedule such conversations to reflect time differences. If appropriate, to ensure minimising the time spent in the UK, we will book consultations and surgery in advance and can help our patients choose suitable accommodation close to our hospitals.
It is very rare to need specialist overnight care for our patients outside of the hospital environment, but this can be arranged if required to suit an individual’s needs. Most patients can undergo surgery and go home within an approximate 2 week period, but we can advise on the time required to stay in the UK on a case to case basis.

Only in exceptional cases involving significant functional impairments or severe structural abnormalities might he consider evaluating younger patients.
